Waiver and Release Agreement

In consideration for being allowed to participate in activities held at, but not sponsored by, Mid-America Baptist Theological Seminary, I agree to the following waiver and release: I acknowledge that participation in the gymnasium has inherent risks for anyone, which cannot be eliminated.

I hereby release, discharge, indemnify and agree to hold harmless Mid-America Baptist Theological Seminary, its directors, officers, and employees, agents, and all volunteer personnel from any and all liability for personal injuries and/or damage(s), injury, or illness that I may suffer.

I further agree to indemnify and hold harmless Mid-America Baptist Theological Seminary, its directors, officers, employees, agents, and all volunteer personnel for any claim and/or damages it, or its agents are required to pay as a result of any injury or damages including reasonable attorney fees, litigation expenses, and court costs.

I’ve carefully read, clearly understand, and voluntarily sign this waiver and release agreement.

In consideration for the above minor being permitted to participate in the activities held at, but not sponsored by, Mid-America Baptist Theological Seminary, I agree to the following waiver, release, and indemnification:

The undersigned parent, guardian, or custodian of the above minor, for himself/herself and on behalf of said minor, hereby joins in the foregoing Waiver and Release and hereby stipulates and agrees to save and hold harmless, indemnify, and forever defend, Mid-America Baptist Theological Seminary, their directors, officers, agents, employees, and volunteers from and against any claims, actions, demands, expenses, liabilities (including reasonable attorneys' fees), and negligence made or bought by said minor or by anyone on behalf of said minor, as a result of said minor's participation in the activities of the event and his or her use of the property, equipment, tools, and facilities of Mid-America Baptist Theological Seminary.
